This is a prayer from the Siddur, recited by the congregation on behalf of sick people. The text reads as follows:

He who blessed our fathers Abraham, Isacc and Jacob, and our mothers Sarah, Rebecca, Rachel and Leah, may he heal ... (name of the sick person) who is ill. May the Holy One, blessed be he, be filled with mercy to cure and heal, revive and sustain him/her, and speedily send him/her perfect health both in body and spirit; and let us say, Amen.

(I will write in calligraphy the name of the sick person in the blank space. Please send me the Hebrew name of the sick person and his/her parents, for example: Sarah daughter of Moshe and Chanah, or Ya’akov son of Yosef and Miriam).
